Registry Programs Fee Schedule
The AAR Policy describes the quality system requirements that must be met for the organization and its affiliated analysts to qualify for registration. An organization should ensure that they can meet the requirements of the policy prior to submitting an application.

The AAR Application includes the forms for the application of the: Initial Organization, Triennial Organization, Initial Analyst, Transfer Analyst and the Drop of Analysts.

Completion of AAR Analyst ADD forms including the quality control information on Form 9 are required to add or transfer an analyst. All attachments and requested information must be submitted for every analyst addition or transfer demonstrating work with their current affiliated organization.

To remove an analyst from your organization, complete and submit the appropriate AAR Analyst DROP forms via email, fax or post.. Form 11 is designed for an organization to drop an analyst. Form 12 is designed for an analysts to drop him/herself from an organization. Submission of both Forms 11 and 12 are not required.

FMR Organization Application
The FMR Application includes the forms for the application of the: Initial Organization, Biennial Organization and Operator Addition. Also included are forms for Personnel Change, Operator Drop and Organization Drop. Follow the General Application Instructions on pages 3-4 to complete and submit the application.

FMR Operator Enrollment Forms
The FMR Operator Add Forms are forms 8-11 of the FMR Application; see complete application document for instructions. The forms must be submitted to enroll an operator with a registered organization. New organizations must complete the FMR Application.

The FMR Operator Drop Form is form 12 of the FMR Application; see complete application document for instructions. Complete and submit the form via email, fax or post to remove an operator from your organization.

FMR Personnel Change Forms
The FMR Personnel Change Forms are forms 2, 3 and 11 of the FMR Application; see complete application document for instructions. The forms must be submitted, with attachments listed on Form 11, to make changes to key personnel within a registered organization: Technical Manager, Quality Manager or Radiation Safety Officer.

Registry Programs Fee Schedule
The Registry Programs Fee Schedule includes the fees and Enrollment Forms for our registry programs. The Fee Schedule also includes the AAT Retest Order Form and the AAT Stock Sample Order Form.
